SHIM Questionnaire
The SHIM questionnaire helps us understand erectile function. Please complete it before your appointment.
The SHIM questionnaire is an abridged version of the International Index of Erectile Function (IIEF) survey. It helps clinicians diagnose the presence and severity of ED in an objective manner. It helps to identify ED and and then monitor for any improvement.
